Overview

Sinestrea — Assassin Guide

Sinestrea is the scariest jungler on the Thai server right now, and a 74.5% ban rate says enemies would rather never see her than work out what she is doing. She is not a clean dive assassin. She is a resurrection-loop predator who weaponizes her own death, literally. Everything runs on Blood Bramble stacks: collect 7 through kills and assists, trigger Blood Ritual to blow yourself up, deal physical damage in a ring around you, and come back at full HP. Keep that stack count ticking and she never really leaves a fight, she just resets it. What makes her especially nasty this patch is how W's range-toggle plays with the current Omni Arms and Claves Sancti itemization, which rewards her on-hit burst in both melee and marked-shot range. Q gives her the stealth repositioning to slip mid-fight and line up a burst auto, and R equalizes HP to punish any squishy sitting above 50%. She is genuinely Hard. Blood Bramble management, the W-mode switching, and the timing on Blood Ritual revival all demand real game sense. Put in the reps at Diamond and above and you get an S+ jungler most enemies simply cannot account for in champ select.

Strengths

  • +Blood Bramble's resurrection mechanic makes Sinestrea functionally impossible to permanently kill in extended teamfights, forcing enemies to either disengage or commit follow-up damage into an already-resetting threat.
  • +W's dual-mode on-hit scaling means Sinestrea deals meaningful damage at any HP threshold, missing-HP melee damage punishes tanky targets, while current-HP ranged damage shreds healthy carries before the melee followup.
  • +Q's stealth-in-place passive trigger gives Sinestrea self-peel and re-engagement that most assassins lack, letting her survive dive attempts and reposition without spending a blink-type summoner spell.
  • +R's HP equalization synergizes with the current high-burst itemization to one-shot squishy targets in under a second when the full combo lands, making her the premier backline-access jungler in the current S+-tier meta.

Weaknesses

  • Stack dependency is a hard constraint: a Sinestrea who has cleared only one or two camps and ganked unsuccessfully enters a teamfight with no Blood Ritual safety net and is simply a squishy, suicidal hero.
  • The 0.5-second Blood Samsara channel is a genuine interrupt window, hard crowd control from Maloch, Baldum, or Grakk will cancel R entirely and leave Sinestrea caught in melee range with her cooldowns burning.
  • Q's stealth is countered by detection-area ultimates and this is not a niche issue on the Thai server, where Lauriel and Kriknak remain high-presence picks that directly nullify her primary repositioning tool.
  • Early jungle clear before Soulreaver is slow and punishable; an opposing jungler who invades at level two or three and forces a skirmish before she has stacks will deny the snowball she needs to function at full capacity.

Abilities

PASSIVE

Blood Bramble

Item active skill is replaced by Blood Ritual. Blood Ritual: Activate to detonate and deal physical damage in an area after 3 seconds. If you have 7 Blood Bramble stacks, resurrect with full HP instead. (Blood Bramble stacks are gained from kills and assists, and are consumed entirely when Blood Ritual is used.)

Blood Hide ability icon1
SKILL 1

Blood Hide

Dash in a chosen direction and become hidden, causing your next attack to be a piercing strike. Passive: After standing still for 2.5 seconds, enter Sleep mode (become hidden).

Blood Ghost ability icon2
SKILL 2

Blood Ghost

Toggle between melee and ranged attack modes. Melee: Deal bonus physical damage equal to 4% of target's missing HP and restore HP. Ranged: Attacks cost 4% of your current HP and apply Bloodstain for 6 seconds. When canceling ranged mode, you can activate Bloodstain to attack a chosen location or apply it to yourself to restore HP.

Blood Samsara ability iconR
ULTIMATE

Blood Samsara

Channel a blood ritual for 0.5 seconds, equalizing your HP and target's HP, then restore additional HP to yourself. Passive: When HP is below 40%, gain 30% movement speed.

How to Use Sinestrea's Kit

P
Blood Bramble

Stack management is the entire game with this passive, never trigger Blood Ritual at fewer than 7 stacks unless you are about to die anyway, because a wasted self-destruct with only 4 stacks is simply dying. The damage pulse on detonation is wide and can finish off low-HP targets fleeing in a teamfight, so position yourself centrally before triggering it. A common high-rank mistake is popping Blood Ritual at full HP; save it for moments when you are already below 30% so the full-HP revival represents maximum swing value.

1
Blood Hide

The dash direction and the subsequent stealth-in-place are two separate tools and treating them as one is how average Sinestrea players get punished, dash for positioning, then stand still for the 2.5-second passive stealth trigger to charge your next empowered lunge attack. The empowered auto after stealth deals significantly amplified damage and should always precede your R in the all-in combo; walking up and ulting without the stealth-buffed auto first is leaving your best damage on the table. Note that Q's stealth is canceled by area-detection abilities (Lauriel R, Kriknak Q), so identify those heroes and adjust your entry angle.

2
Blood Ghost

This is Sinestrea's most mechanically demanding ability and the one that separates good players from great ones. Lead on a high-priority target with ranged mode to apply the Bloodstain debuff, the 4% current-HP drain chip damage alone on a full-HP marksman is enough to tilt the trade, then swap to melee mode for the sustained missing-HP damage and lifesteal when you close in. The cancel-and-detonate on Bloodstain is especially powerful: toss it onto an escaping target, swap out of ranged mode, then fire the Bloodstain detonation projectile at them from across a wall. Never stay in ranged mode when a target is already below 40%, melee W's missing-HP modifier scales brutally at that threshold.

R
Blood Samsara

The 0.5-second channel before HP equalization means you must not cast R while crowd-controlled or instantly interruptible, flash or Q-dash to a safe angle first. Against marksmen and mages sitting above 70% HP, the equalization alone half-kills them before your combo finishes, which is why Sinestrea deletes backline carries so efficiently. The passive 30% movement speed bonus at sub-40% HP on this skill is not a throw-away line: use it to dodge skill shots during your Blood Ritual revival animation rather than standing still and eating damage.

Strategy

Gameplan

Power Spikes

Sinestrea's first spike is Soulreaver at level 4, where Q's full stealth and W's range toggle are both online at once. She gets oppressive the moment Broken Spear comes in as her third item: the armor shred plus Omni Arms' on-hit proc kills squishies faster than they can respond. She is weakest in the first two jungle clears before stacks build, and stronger with every kill after that. Late game with Claves Sancti she one-taps anyone under 50% HP, and Blood Ritual turns every 5v5 into a second-wind nightmare for the enemy.

Early Game

Levels 1–6
  • Start Blue Buff then rotate to Red, prioritizing a full clear to level 4 before your first gank — Sinestrea's pre-Soulreaver damage is not reliable enough to justify an early level-2 invade unless the enemy jungler is completely overextended.
  • Use Q's passive stealth (stand still 2.5 seconds in a bush near the target lane) rather than burning the dash itself on approach for ganks — arriving with Q's dash still available gives you a second gap-close if the target flashes.
  • Track your Blood Bramble stacks obsessively from minute one; your first gank kill is effectively worth two resources — gold AND your first stack, making successful early ganks geometrically more valuable than on most junglers.

Mid Game

Post Broken Spear
  • At three items (Soulreaver + War Boots + Broken Spear), start forcing picks on isolated enemy carries roaming through the river — you can delete a squishy target in under 1.5 seconds and the Blood Ritual threat alone will deter enemies from counterchasing.
  • Shadow the allied carry's rotations to Turtle or the first Dragon, using Sinestrea's dueling and pick potential to threaten the enemy jungler away from contesting objectives rather than face-fighting 5v5 before your team is ready.
  • Build stacks actively by participating in every skirmish you can reach, even if it means abandoning the jungle camp you were clearing — arriving at a fight with 5+ stacks is worth far more than a completed camp that gives you zero Blood Bramble progress.

Late Game

Teamfight phase
  • Identify the enemy ADC or mid-lane mage as your Blood Samsara target before every teamfight and communicate it — your job is not to frontline but to delete the backline carry with one R-combo and then use the Blood Ritual revival to extend the fight.
  • Never enter a late-game teamfight with fewer than 5 stacks if you can avoid it; if you are at 3 stacks, spend 20 seconds farming two jungle camps to hit 5 before committing — the resurrection safety net changes your entire risk calculus.
  • In the final push, use the sub-40% HP movement speed from R's passive to slip through choke points that would trap other assassins, and position yourself as close to the enemy core as possible before Blood Ritual detonates so the damage pulse hits multiple enemies.
Matchups

Matchups

Her S+ status makes her a must-ban in organized Diamond play, so hide the Sinestrea pick until the last rotation or bait the ban with a decoy flex. If Lauriel or Kriknak are open and unbanned, think hard about banning one yourself, because getting revealed mid-dive throws away the whole plan. Against Maloch and Baldum, don't fight through them. Work out who they are protecting and route around it: dive the mage, not the tank. If the lane structure allows it, draft Grakk or Arum before locking her, because a guaranteed R land takes her from strong to unplayable-against.

Sinestrea gets countered by

  • Lauriel hero icon
    Lauriel
Lauriel's R creates a massive detection aura that reveals and cancels Sinestrea's Q stealth, removing her primary engagement tool and leaving her exposed in melee range with no re-entry option.
  • Kriknak hero icon
    Kriknak
Kriknak's Q area-detection reveals stealthed units and his overall lockdown chain interrupts Blood Samsara's channel, making him able to out-jungle and out-duel Sinestrea in the early skirmish window she depends on.
  • Maloch hero icon
    Maloch
Maloch's point-and-click R interrupts Blood Samsara's 0.5-second cast and his sustained tankiness means Sinestrea's current-HP and missing-HP modifiers both underperform against his bulk, forcing her to spend stacks she cannot afford.
  • Baldum hero icon
    Baldum
Baldum's R throw hard-interrupts Blood Samsara before it resolves and his ability to peel for the backline means Sinestrea's backline-access gameplan requires burning Q and passive stacks just to reach her target, depleting her resources before the real fight begins.

Sinestrea synergizes with

  • Grakk hero icon
    Grakk
Grakk's chain pull drags the enemy backline carry out of position and into Sinestrea's melee range, eliminating the entire gap-close problem and guaranteeing the Blood Samsara equalization lands without the target being able to move.
  • Natalya hero icon
    Natalya
Natalya's R area denial locks down the backline while Sinestrea dives in, and Natalya's raw poke damage pre-softens targets below the HP threshold where W melee's missing-HP modifier becomes lethal in a single auto.
  • Annette hero icon
    Annette
Annette's knockback and shield provide both a setup and a survival tool for Sinestrea post-dive, letting her survive long enough in the enemy backline to trigger Blood Ritual and come back on a second rotation.
  • Arum hero icon
    Arum
Arum's pull-and-hold setup gives Sinestrea a near-guaranteed Blood Samsara land on a stationary target, turning what is normally a skillful mechanical requirement into a free kill in organized play.
Tips

Pro Tips

  • Learn to 'bank' your Blood Bramble trigger: if you hit 7 stacks mid-jungle-clear rather than mid-fight, hold Blood Ritual until you are actively in a teamfight at low HP so the revival creates maximum chaos, a Blood Ritual pop in an empty jungle is a wasted stack reset.
  • W's Bloodstain detonation is not just a chase tool, you can apply it to yourself to heal in an emergency by casting the return-fire on your own position, which experienced players use to stay healthy through jungle clear without recalling between rotations.
  • The Q dash direction is independent of your facing direction: you can fire Q away from a target to trick an enemy into thinking you're retreating, then stand still for 2.5 seconds in a brush to passive-trigger stealth and lunge-auto them from an unexpected angle.
  • Against heroes with delayed-burst ultimates (Tulen, Ilumia), deliberately drop below 40% HP before the teamfight starts to pre-activate R's passive 30% move-speed bonus, arriving at the fight pre-loaded with mobility makes her nearly impossible to land slow skill shots on.

Sinestrea is for experienced junglers who are comfortable juggling resources and want a hero that can't be out-valued in long fights. The thing to master is Blood Bramble timing: knowing exactly when to detonate Blood Ritual for max fight impact versus holding it is the gap between a 55% and a 70% win rate on her. She rewards deep investment and punishes casual play, which is why that ban rate is fear, not frustration. Learn the stack loop and you have arguably the best jungler on the server this patch.

FAQ

Sinestrea — Frequently Asked Questions

Is Sinestrea good in the current ROV patch?+

Sinestrea is rated S+ tier on the Thai server in the current patch, with a 56.6% win rate and a staggering 74.5% ban rate in Diamond-and-above ranked play. She is arguably the most feared jungler in the meta precisely because her Blood Bramble resurrection mechanic makes her difficult to permanently deal with even in losing fights. If she isn't banned, she almost always wins.

What is the best build for Sinestrea?+

The core build is Soulreaver into War Boots, then Broken Spear, Omni Arms, Claves Sancti, and Blade of Eternity as the six-item full build. Broken Spear is the critical third-item power spike, the armor penetration dramatically amplifies both W's on-hit modes and the Blood Samsara combo damage. Blade of Eternity in the sixth slot creates a second layer of revival insurance on top of Blood Bramble, which is extremely hard to play around.

How do you counter Sinestrea?+

The most reliable counter is picking detection-based ultimates like Lauriel, which cancel her Q stealth and strip her of the empowered auto she depends on for burst setup. Hard crowd control that interrupts Blood Samsara's 0.5-second channel, specifically from Maloch and Baldum, also dramatically reduces her kill threat. At the draft level, banning her outright remains the single most common answer at high rank, evidenced by the 74.5% ban rate.

Is Sinestrea hard to play / good for beginners?+

Sinestrea is rated Hard difficulty and is not recommended for beginners or players new to the jungle role. She requires simultaneous management of Blood Bramble stacks, W mode-switching, Q stealth timing, and Blood Samsara's interrupt window, mistakes on any one of these in a fight are usually fatal. Players who want to learn the archetype should first develop strong jungle pathing fundamentals on an easier jungler before committing to Sinestrea's mechanical ceiling.
